The Partnership for Healthier Communities is a community health center-led Clinically Integrated Network (CIN) comprised of 19 federally qualified health centers serving more than 385,000 West Virginians across rural and underserved communities.

About the Partnership for Healthier Communities (PfHC)

Through coordinated quality improvement, shared performance data, and aligned value-based contracting strategies, the PfHC builds the infrastructure necessary for community health centers to succeed in an increasingly data-driven and outcomes-focused healthcare environment.

The PfHC supports interoperability and real-time data exchange across participating organizations, enabling actionable insights at the point of care. By leveraging shared analytics platforms, standardized quality measures, and coordinated care management strategies, the network strengthens connectivity among providers and promotes seamless transitions across care settings. This integrated framework enhances primary care coordination, supports proactive population health management, and advances the development of a statewide “connected care” approach.
