Job Description:
Community Care of West Virginia is seeking a full-time primary care provider for our Slaty Fork clinic near Snowshoe Mountain. This outpatient position provides team based primary care to patients of all ages in a rural community setting.
Responsibilities:
• Provide outpatient primary care/urgent care
• Deliver quality patient centered care
• Work collaboratively in a team based clinical model
• Maintain accurate and timely documentation

This position will work primarily between Logan and Mingo Counites in West Virginia. ACT Case Manager serves a population of chronic mentally ill, some of whom have dual diagnoses, by providing the basic core services of case management (including service planning, linkage, referral, assessment, advocacy, and crisis support as needed).

Mountain Laurel Integrated Healthcare (MLIH) is seeking a Team Leader for our ACT (Assertive Community Treatment) Program.
