Collecting Social History and Risk Factors


Event Description: The West Virginia Primary Care Association (WVPCA), in collaboration with the HITEQ center, invites you to a two-part virtual training focused on strengthening social needs data collection across West Virginia’s community health centers. Designed for clinical, care coordination, and quality improvement staff, this training will explore practical strategies for effectively collecting and using social needs data.

Part One held on May 20th will offer a foundational overview of why social needs data collection matters, how to introduce and ask questions in a patient-centered way, and ways to overcome common barriers to implementation. Attendees will explore common tools such as PRAPARE, learn from peer examples, and consider how platforms like Azara can support data use.

Part Two held on June 3rd will shift to the application of this data—how to close the loop on referrals, leverage community-based resources, and activate care team workflows. This session will also include a spotlight on FindHelpWV, the platform being launched for the WVPCA health center network, and discuss how it can be integrated into existing referral processes.

Objectives and Goals: 

  • Understand the importance of social needs data collection and identify how this information can improve patient care, health outcomes, and organizational goals.

  •  Build confidence in patient-centered screening techniques by learning how to introduce and ask social needs questions in a way that builds trust and minimizes discomfort.

  • Explore strategies to act on social needs data, including referral workflows, partnerships with community-based organizations, and the use of tools like FindHelp to support closed-loop referrals.

Target Audience: Quality staff, front office staff, care management staff

Cost: This training is being offered at only $25 per participant!
