Please join the WVPCA as we are excited to announce "PRAPARE SDoH Implementation Webinar Series" in partnership with Yuriko de la Cruz, Social Determinants of Health Manager, National Association of Community Health Centers. We will be hosting two webinars focusing on the social determinants of health, using PRAPARE, and resources from WVPCA and the National PRAPARE Team. Please note this webinar series replaces the previously cancelled April 14th PRAPARE webinar and ALL attendees will need to register for this new event.

Target Audience:  Quality Improvement Leadership, Nurses, Medical Assistants, Community Health Workers, Care Managers, Patient Navigators, Care Coordinators, Social Workers and anyone who screens and/or refers SDoH.

There is no cost for registration.
 
Webinar 1:  Overview of the Social Determinants of Health and Collecting Social Risk Data Using PRAPARE

Topics to be Covered:  
  • Define the social determinants of health and the impact of patient health outcomes. 
  • Explore how social risk data collection aligns with national and payer-related priorities.
  • Understand what PRAPARE is and the use of it to collect social risk data of patients.
Webinar 2:  Closer look at PRAPARE Workflows and Resources to Address Social Risk Needs

Topics to be Covered:  
  • Explore examples of how PRAPARE has been implemented.  
  • Discuss challenges and mitigation strategies to implement PRAPARE.
  • Identify resources and tools from WVPCA to support CHC’s with addressing social risk needs.
 Registration closes June 10th at noon.

CE Information: The WVPCA is accredited as a provider of Continuing Nursing Education by the West Virginia Board of Nursing (WVPBRN Provider #50-26228). CE credit hours will be available pending final approval.
