Please join the WVPCA along with Dr. Matt Weimer, Chief Medical Officer at Valley Health Systems, Brett Wellman, FNP-C, Valley Health Systems, and Johnna Beane, Health Strategies Coordinator from the American Heart Association as they present the Target:BP Program and Million Hearts Initiative. Participants will learn how Valley Health Systems has improved their hypertension control rates by implementing the American Heart Association Programs. Participants will learn about Valley Health Systems redesigned workflow and staff training to improve blood pressure measurement accuracy and much more.

Participants will be able to:

  • State the requirements to join the AHA and AMA Target: BP hypertension quality improvement program
  • List 3 strategies to be used to improve hypertension control in the health center
  • State how partnerships can assist health centers improve hypertension outcomes

CE Information: The WVPCA is accredited as a provider of Continuing Nursing Education by the West Virginia Board of
Nursing (WVPBRN Provider #50-26228). CE credit hours will be available pending final approval.
 

Target Audience: Providers, QI Staff, Nurses, Medical Assistants and Community Health Workers
