The West Virginia Primary Care Association (WVPCA), in conjunction with Jennifer Calohan of CURIS Consulting, is hosting the third of six monthly Learning Collaborative Webinars. This webinar, scheduled for Tuesday, December 18, will discuss all aspects of the Care Plan, including the requirements for CMS Chronic Care Management and PCMH. 

During the webinar, participants will have an opportunity to share questions, ideas and comments about Care Planning to promote peer-to-peer learning.

OBJECTIVES:

  • Participants will have the ability to identify required elements of a care plan.
  • Participants will understand that a care plan is patient-centered.
  • Additionally, attendees will gain the knowledge to develop care plans that meet CMS Chronic Care Management and Patient Centered Medical Home guidelines.
     

TARGET AUDIENCE:

Any CHC staff member who is or will be part of a care team including:
  • Primary Care Providers
  • Nurses
  • Care Managers/Coordinators
  • QI Staff
