The West Virginia Primary Care Association (WVPCA), in conjunction with Jennifer Calohan of CURIS Consulting, is launching the first of six monthly Learning Collaborative Webinars. The first webinar, scheduled for Tuesday, October 30, will provide West Virginia Community Health Center (CHC) staff with the knowledge and tools to form High-Performing Care Teams and keep them engaged. This interactive webinar will allow CHC care team members to ask questions and hear from their peers. Participants will also learn the core requirements for NCQA PCMH Team-Based Care.
During the webinar, participants will gain an increased understanding of Care Teams and learn strategies to egage care team members in the concepts and criteria of NCQA PCMH.
Any CHC staff member who is or will be part of a care team including:
- Medical Assistants
- Care Coordinators/Care Managers
- Quality Improvement Staff
