There are currently 33 CHC organizations providing care to nearly 1 in 4 West Virginians at more than 390 plus practice locations statewide. We are the largest primary care network in the state. With a long standing commitment to quality care, percent of West Virginia CHCs are recognized by NCQA as Patient Centered Medical Homes. Our clinically progressive facilities offer sophisticated medical technology, including electronic health records, pharmacies, lab services, dental care and behavioral health care, all delivered in a team-based, integrated care model.
