About The Forum


Please join the WVPCA for the 2023 Community Health Center (CHC) Leadership Forum, themed to reflect our updated organizational vision: Leading the Way to A Healthy West Virginia. This three-day event will bring together administrative and clinical leadership, key services staff, and your PCA team for a dynamic and collaborative series of speakers and educational sessions. 

Held at the scenic Stonewall Resort Monday September 11th - Wednesday, September 13th, the WVPCA Leadership Forum will highlight state and federal partner healthcare priorities, innovative quality improvement strategies, best practices in workforce development, value-based contracting, and more.

Honoring tradition, the WVPCA Leadership Forum is being held at the Stonewall Resort in Roanoke, WV. We have reserved a block of rooms at a discounted rate for those attending the CHC Leadership Forum. Space is limited so don't wait to call!

From the South: From Charleston area, follow I-79 north. Take exit 91 towards Roanoke on Rt. 19. Follow Rt. 19 to the destination, noting signage to access the Resort Lodge.
From Beckley/Princeton area, follow Rt. 19 north to I-79 north. Exit at 91, towards Roanoke on Rt. 19. Follow Rt. 19 to the destination, noting signage to access the Resort Lodge.   

From the East: Head West on I-64 towards Beckley. At I-64/I-77, take the North Beckley/Summerville Exit onto US-19. Head north on US-19 to I-79 north. Exit at 91, towards Roanoke on Rt. 19. Follow Rt. 19 to the destination, noting signage to access the Resort Lodge. 

From the West: 
Head East on US-64 towards Charleston. Take exit 59 for I-77 N toward I-79/Parkersburg. Follow I-79 north. Take exit 91 towards Roanoke on Rt. 19. Follow Rt. 19 to the destination, noting signage to access the Resort Lodge.



2023 Health Care Awards


The West Virginia Primary Care Association wants to honor your health center staff for their exceptional gifts of time, effort, and dedication to the Community Health movement. The WVPCA Annual Award program begins with you! Award winners will be announced at this year's WVPCA CHC Leadership Forum in September. 

The WVPCA uses a nomination review rubric to determine the award winners. Nominations are given points based on these criteria:
  • Relevance of the nomination to the category
  • Number of nominations received for that individual
  • Commitment and dedication to health center and community
  • Commitment and impact on patients
  • Compelling story of why the nominee should be given the award
  • Must be at least one paragraph
  • Number of years worked at the health center

Distinguished Service Award

The Distinguished Service Award recognizes significant lifetime contributions and service to the Community Health Center system of care in West Virginia. Those eligible include individuals in an administrative role within WVPCA member organization.
Nominations can be submitted by anyone at the community health center level. 

 

Pamela J. Moore Health Center Engagement Award

This award, named in honor of Pamela J. Moore, recognizes outstanding engagement in; contributions to; and commitment to our network of community health centers.

Pam Moore was a longstanding member of the WVPCA team, supporting member engagement as well as PCA and CHC collaboration, and the events and trainings provided to bolster best practices and partnership at the community level. With over 35 years in support of community health centers, Pam was unapologetic in her role and understood the critical need for deep connection and relationship building. As she would say – "It’s the people that matter!"


Individual community health centers within the WVPCA who engages with the Association and its network consistently through points of engagement are eligible for this award. Engagement points including but not limited to: PCA trainings, events, grant opportunities, pilot programs, innovative opportunities, and participation in collaborative spaces such as Noddle Pod, ListServs and monthly affinity group calls. Selection is made by WVPCA staff, utilizing anecdotal and data driven engagement points. 

 

Outstanding Advocacy Leader Award

The Outstanding Advocacy Leader Award recognizes an individual within one of the Association member organizations who has made an impact on both their community health center and their community/patients through participation in advocacy-related efforts at either the state and/or federal level or through grassroots advocacy work. Grassroots advocacy is about finding strength in numbers — and harnessing that strength to influence public opinion and government action by engaging employees, members, patients, and other stakeholders to raise awareness and empower individual voices.

Nominees may include, but are not limited to, front-line staff, outreach workers, financial assistance staff, interpreters, accounting, and workforce staff who have contributed to promoting patient care and/or health center processes through advocacy. Nominees demonstrate a commitment to the mission of the health center and a dedication to amplify the role and visibility of Community Health Centers within the local community and beyond.

Nominations can be submitted by anyone at the community health center level. 


Outstanding Provider Award


The Outstanding Provider Award recognizes a distinguished provider who exemplifies excellence in clinical practice and demonstrates a dedication to the Community Health Center mission and to the patients they serve. Those eligible include anyone who is a medical, dental, or behavioral health provider at a West Virginia Community Health Center.

Nominees may include but are not limited to physicians, dentists, nurse practitioners, physician assistants, dental hygienists, registered nurses, nurse midwives, social workers, and pharmacists. Nominations can be submitted by anyone at the community health center level.
