In 2012, the WVPCA and its membership ratified an initiative to “Build a Culture of Continuous Quality Improvement” within all community health centers. With the impending federal reimbursement changes, emphasis on patient-centered medical homes, and need to change the care model for high risk patients, this initiative is critical to our long-term sustainability. “Building a Culture of Continuous Quality Improvement” will be supported by training and technical assistance in four key areas: leadership, finance, data and process improvement. The Care Team Management workshop series is part of our overall strategy and will focus on leadership and process improvement. This topic has been chosen for the first program because of its relevance and importance to the West Virginia Family Health Plan although the workshop content is appropriate for all patient populations.
Each health center is encouraged to send clinical teams from their sites, including medical and executive directors. We hope to have a good mix of medical and behavioral health providers, administrators, pharmacists, nurses/medical assistants, and health coaches.
Care Team Management/Managing Complex Patients will be offered in two convenient locations, Feb. 7 at the Holiday Inn & Suites in South Charleston, and on Feb. 8 at Lakeview Resort in Morgantown. There is no fee to attend.
These workshops are designed to promote learning across our network of health centers with outside partners and experts; and to explore the challenges faced by clinicians in their work with patients while enhancing skills.

This event is open to community health center CEOs, CMOs, PAs, NPs, nurses, medical assistants, BH providers, and health coaches.
